Financial Policy

Patient Billing

For your convenience we accept Visa, MasterCard and Discover. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at Yulee Family Dentistry Phone Number 904-225-4999. Many times, a simple telephone call will clear any misunderstandings.

Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan but this must be done prior to the actual procedure.

BROKEN APPOINTMENT POLICY

Trust and respect are very important to the Dentist-Patient relationship.  We pledge that we will always respect the time reserved to patients for their dental needs, and patients can trust that we will never cancel or remove their reserved appointment time without notice.  In return, we ask that patients respect our time and we can trust that patients will honor the reserved appointment times as well.  We know things come up in life and sometimes circumstances cannot be avoided.  Therefore, the following is the office policy for less than 48 hour cancellations and no shows:

1st offense: the office will allow one waived broken appointment. A credit card will be required to be kept digitally on file for future offenses. No future appointment can be reserved until the credit card is on file.

Future offense: a 50$/hr of reserved time will be charged before future appointments can be made.  Excessive or repeat broken appointments/no shows will warrant release of the patient from the dental practice. Any patient released from the dental practice will be given 30 days of emergency coverage should any emergencies arise in the transition time period.
